PARENT CONSENT

I give permission for Citipointe Church to obtain emergency  medical, hospital or ambulance service at any time they consider necessary. I acknowledge that I will be liable for any medical, hospital or ambulance expenses incurred in the treatment for my highschooler. Citipointe is released from any and all liability in the event of any accident or misfortune, damage or loss that may occur to the highschooler.

Citipointe may take Photographs / Video footage of the activities at Youth Camp.  Citipointe reserves the right to use this material for promotional purposes. 

No highschooler will be allowed to leave the camp venue until they are picked up by the parent/guardian.  Citipointe Church, its employees, and volunteers as organizers of Citipointe Church take no responsibility for any child leaving the supervised site for whatever reason.
