1. I agree to observe and respect all policies, procedures, terms and conditions of being a Team Member as provided to me during induction and orientation and as varied from time to time.

2. I hereby declare all information given by me in this application is true and correct in every detail.

I agree that other than those matters disclosed by me to Citipointe at the time of making this declaration, I know of no past behaviour that renders me unfit to serve as a volunteer with children or which detracts from the obligation of the church to operate as a place of safety to a minor or any other persons.
