We are excited you are interested in joining our Student Team at Youth Society. We believe that God has something incredible in store for your life! Not only is this one of the most life impacting decisions to make, it is also a bunch of fun!

Joining the team is easy. All you have to do is sign up and complete our 3 week team program. Once you complete this, you will graduate onto the team!

STATEMENT BY APPLICANT

By completing this Application, I agree to the following.

1. I authorise Citipointe to contact my nominated referee as required.

2. If my application is successful, I will be bound by and at all times, observe and respect all policies, procedures, terms and conditions of being a Team Member as provided to me during induction and orientation and as varied from time to time.

3. I hereby declare all information given by me in this application is true and correct in every detail.

4. Other than those matters disclosed by me to Citipointe at the time of making this declaration, I know of no past behaviour that renders me unfit to serve as a volunteer with children or which detracts from the obligation of the church to operate as a place of safety to a minior or any othe persons. I have not knowingly withheld any circumstances or facts that would, if disclosed, affect my application.

5. Citipointe will hold the information in this application. I understand that this information will be kept in a confidential manner and used only for screening and disciplinary purposes.
