We look forward to you joining Team with us here at Citipointe Kids. It is so rewarding and fun to look after and guide the next generation to Jesus. From running around with the little ones to having conversations with the big kids or even vaccuming after the service... We want to make sure you are safe and communicated with when you're on our Team.

WORKING WITH CHILDREN CHECK - THE BLUE CARD SYSTEM

If you are over 18yrs of age, you need a blue card to work with children.

If you have a current blue card, we need your permission to link the Ciitpointe organisation to your card. Your Kids Pastors will advise you on how to do this.

If you do not have a current blue card you will be required to get one before you will be allowed to serve with Kids. Once the application is submitted, it should take between 5 day and 2 weeks to receive notice

STATEMENT BY APPLICANT

By completing this Application, I agree to the following.

1. I declare that I am an active member of Citipointe Church.

2. I understand I must undergo a Working with Children check with the Blue Card services and receive a positive notice before I am able to serve with children or young people.

3. If my application is successful, I will be bound by and at all times, observe and respect all policies, procedures, terms and conditions of being a Team Member as provided to me during induction and orientation and as varied from time to time.

4. I understand that Citipointe Church operates in an environment of numerous legal and ethical restrictions, and I will fully cooperate with Citipointe Church in abiding by these. I assure Citipointe Church of the following, in considering me for this volunteer position within the Families Demographic but not directly working with Children/Youth that:

I have no health impediment that will put me or any other person at risk in the fulfilment of my designated role.
I know of no past behaviour that renders me unfit to serve in the capacity outlined in this Agreement or which detracts from the obligation of Citipointe Church to operate as a place of safety to a minor or any other person. Such past behaviour may include, but not be limited to, being the instigator of sexual abuse (whether convicted or not), including any type of molestation, indecent exposure, child pornography, sexual harassment or intimidation.

5. There is nothing in my background that I have not disclosed to Citipointe Church, which if it becomes public knowledge could adversely affect the assessment of my good name and character.

6. Other than those matters disclosed by me to Citipointe at the time of making this declaration, I know of no past behaviour that renders me unfit to serve as a volunteer

7. I understand that if I am unclear as to any of the statements in this Agreement, I will seek clarification from Citipointe Church before signing.

8. I understand Citipointe will hold the information in this application. I further understand that this information will be kept in a confidential manner and used only for screening and disciplinary purposes.

9. I hereby declare all information given by me in this application is true and correct in every detail.

10. I understand that any material misstatement in or omission from this Agreement and declaration may render me unfit to be a participant in working with children or young people at Citipointe.
