TEAM AGREEMENT

I consent to the information contained in this Agreement, including the subsequent pages, to be kept by Citipointe. I understand that this information will be kept in a confidential file and used only for screening and disciplinary purposes.

I have been provided with, or have access to, and agree to abide by the specific Citipointe’s policies that relate to my volunteer position when undertaking work or services for Citipointe including but not limited to the Code of Conduct, and Health and Safety Procedures.

I declare that I have been an active member of my local church for more than 6 months (for Life Group Leaders).

I understand that Citipointe operates in an environment of numerous legal and ethical restrictions, and I will fully cooperate with Citipointe in abiding by these. I assure Citipointe of the following, in considering me that:

I have no health impediment that will put me or any other person at risk in the fulfilment of my designated role.

I know of no past behaviour that renders me unfit to serve in the capacity outlined in this Agreement or which detracts from the obligation of Citipointe to operate as a place of safety to a minor or any other person. There is nothing in my background that I have not disclosed to Citipointe, which if it becomes public knowledge could adversely affect the assessment of my good name and character.

I agree and consent to participating in the Citipointe program and activities. I hereby also agree to release, discharge and to hold harmless Citipointe for any accidents, harm or loss, howsoever sustained, regarding participation in Citipointe programs or activities.

I understand that if I am unclear as to any of the statements in this document, I will seek clarification from Citipointe before signing.

I have provided this information, and any documents accompanying it in good faith and declare they are true and correct to the best of my knowledge and belief.

I understand that any material misstatement in or omission from this Agreement and declaration may render me unfit to be a participant as a volunteer.
